Insurance companies follow strict guidelines to determine if bariatric surgery is a covered benefit. Individual insurance carrier requirements for submission of the prior authorization request package vary significantly. These requirements may be waived under certain circumstances. oBand insurance authorization specialists will maximize your chances of timely prior authorization.

Our Services

Authorization Request

A package which carefully addresses your individual insurance carrier/plan submission requirements will be assembled on your behalf. This package will clearly establish that significant weight loss is medically necessary and surgery is your only option.
 

Identification of Individual Circumstances

Even if your BMI/weight, medical problems, and failed weight loss attempts do not fully meet the standard requirements, extenuating circumstances will be identified on an individual basis.
 

Letter of Medical Necessity

A series of consultations with our multispecialty medical panel will be coordinated for you as needed. If you are a candidate, the panel will write a comprehensive letter of medical necessity with a strong recommendation for obesity surgery.

Appeals

If you are determined to be a candidate by our medical panel but coverage is denied, our expert medical panel will prepare an appeal on your behalf. Our expert attorneys may also get involved.

Financing

If you are not determined to be a candidate for insurance coverage by our expert medical panel, patient financing programs will be made available to you. Some patients qualify for financing programs with payments as low as $300 per month.

Cost

All services above are free of charge for oBand Surgery Centers patients. Applicable fees will be discussed in advance with patients having surgery at other facilities.
