Our 30-minute evaluations test your vitals to determine if you pass the National Institute of Health's requirements for weight-loss surgery. The Free Evaluation is available to patients with PPO and POS insurance plans, and to those who will self-pay or receive financing. If you have a HMO, Medi-Cal or Medicare plan, you may schedule a Free Evaluation if you select the self-pay or financing options.
